How To Claim?

When a need to claim arises you must complete and submit a claim form. The circumstances under which you can claim and the necessary documents required for the claim to be assessed are as follows.

Claim Type	Required Documents From Clients	Forms
Maturities	• Policy document • Identity Card or Passport of owner • Payments are not made to persons less than 25 years. The claimant should be the owner.	• Maturity Request Form
Encashment	• Identity Card or Passport	• Encashment Form (Mompati) • Encashment Form (BIFM) • Encashment Form (Tshireletso)
Loans	• Policy Document • Identity card / passport	• Loan application Form
Death Claims	• Death certificate • Policy document • Out Patient Medical record for 2 years prior to the commencement date of (life cover) policy. • Identity Card of the deceased & of beneficiaries. • Birth Certificates where the beneficiary is a minor and does not have an ID.	• Declaration of identity Form • Claimants Declaration Form
Accidental Death Claim	• See Death Claim – conditions are similar. • Drivers License if the deceased was the driver. • Inquest & Postmortem Reports from Medical Practitioner. • Police and Witness report.	• Declaration of identity Form • Claimants Declaration Form
Funeral cover for Family Funeral Benefit or Extended	• Death notification from a Medical Practitioner. • Death certificate or Notice of Death from a local Chief accompanied by an affidavit. • Identity card of the deceased. • Identity card of the claimant(s) / beneficiary(ies) NB: there is a waiting period of 6 months from date of commencement.	• Affidavit (for Death at home) • Funeral Benefit Claim Form
Disability claim	• Sickness Disability – Doctor’s Certificate. • Accidental Injury • Doctors Certificate • Drivers License if the injured is the driver • Police and Witness reports.	• Disability Form (Completed by Claimant / Disabled) • Disability Form (Completed by Employer
Hospital Cash	• ID / Passport of owner • ID of the beneficiary (dependent) • Hospital invoice • Doctors report	• Hospital Cash Claim Form
Family Commuter Cover	• See Accidental Death claim • Drivers license if the policy holder or beneficiary was the driver. • ID of the policy holder and police report of beneficiary • Doctors Report	• Family Commuter Cover Claim Form
Surrender	• Policy Document • Letter informing us to cancel • Copy of ID/Omang	• Surrender / Cancellation request Form
