How To Claim?

Group Life and or Disability, Permanent Health Insurance, Group Credit and Group Mortgage

• A claim is done preferably through the broker or directly through Botswana Life Insurance in the event of direct clients.

For one to claim the following documents are required.

In the event of a Funeral Claim

• Group funeral claim discharge form
• Group funeral nomination form
• A certified copy or original of a notification of death or if a person dies at home, a letter from the tribal authority together with an affidavit
• A certified copy of the deceased omang/passport
• A certified copy of the claimant’s omang document
• A funeral claim form fully completed by the claimant obtainable from Botswana Life Insurance.
• A police report in the event of accidental death
• In the case of stillbirth, a copy of the stillbirth certificate
• BOTSWANA LIFE INSURANCE reserves the right to call for any other documentation to ensure that the claim is valid

In the event of a Group Life Death Claim

• Group Life claim discharge form
• A certified copy or original of a death certificate
• A certified copy of the deceased omang/passport.
• A certified copy of the claimant’s omang document
• A claim form fully completed by the claimant obtainable from Botswana Life Insurance.
• A police report in the event of accidental death
• Botswana Life Insurance reserves the right to call for any other documentation to ensure that the claim is valid

In the event of a Group Credit and Group Mortgage Death Claim

• Group Credit Life Claim discharge form
• Death Claim form
• Death certificate or certified copy of death certificate
• Police report(if death is due to an accident)
• Copy of Omang/Passport
• Loan statement(showing the outstanding balance)

In the event of a Group Credit and Group Mortgage Disability Claim

• Group Credit Life Claim discharge.
• Medical attendant statement / report
• Copy of omang/Passport
• Statement by claimant(work history, qualifications & medical practitioners consulted)
• Letter from employer(job description, remuneration details and work history)
• Sick leave records, if any
• Loan statement( showing outstanding balance)
• Botswana Life Insurance is to be notified within 3 months of date of disability)

In the event of a Group Life Disability Claim

• Group Life Claim discharge form
• Personal statement form
• Employers statement form
• Job description( to be completed by employer)
• Confidential medical report(General practitioner)
• Confidential medical report(specialist physician)
• Sick leave certificates
• Salary advise slip
• Claimants omang/passports
